Hello, Parents and Students! Welcome to HESTON! My name is Nurse LaShawna! And it is my pleasure to serve your children and keep them healthy and safe while at school each and every day!

Below are a list of health requirements for students to attend school in the School District of Philadelphia:
- In the event your child needs a medication at school, please see the attached Med-1 form. The prescribing doctor needs to fill out the left side of the form and parents need to fill out and sign the right side.
- Bring the signed forms to me with the labeled medication from the pharmacy.
- ANY and ALL medications that are in school need to have this form signed by the student’s doctor and parent. NO student should have a medication in school without the nurse’s knowledge. Please, it’s a safety issue. Let’s work together to get what everyone needs to keep our students safe!
Proof of Physical Exam is needed to attend school. Proof must be provided on original entry to school for Kindergarten or 1st, and first day of 6th grade.
Proof of Dental Exam is also required. Proof should be provided on entry to school for Kindergarten or 1st grade, and first day of 3rd grade and 7th grade. Please see attached forms. But, as long as documentation is provided from the physician or dentist showing proof of exam, it is acceptable. If you are not sure you have provided proof of medical or dental exam to school, please send in documentation, at any time, so I can update the file!
Also, please fill out and return the attached Student Emergency/Medical Information Form – S865. I need a new one signed at the beginning of each school year!
Immunizations are mandatory to attend the SDP.
- For incoming Kindergarten students, work with your healthcare provider in obtaining the proper sequence of immunizations.
- Two more immunizations are currently required in 7th grade as well! Please see attached flyers for immunization schedule and do not hesitate to reach out to me with any questions or concerns.
- Students on a catch-up schedule need to provide documentation from their treating physician as to the current plan.
- Also, students who are not receiving immunizations due to a medical, religious or philosophical objection need to provide the signed Exemption Form (see attached) every year at the beginning of the school year.
Health Screenings are performed by the school nurse every year. If you do not want your child to take part in health screenings, please just let me know.
Every student gets vision and their height and weight checked every year. Kindergarten through 3rd grade and 7th grade receive hearing screenings. 6th and 7th are also checked for scoliosis. If a student in any grade has a physical exam completed by a doctor that also completed the health screenings, please provide the documentation to the school nurse!
